I, ____________________________, purchaser, agree to take the dog/puppy purchased for the amount of $___________ from Taylor-Made Chihuahuas, seller, on ___________(today's date) to a certified veterinarian of my choice by ______________( 3 days) for a health examination to verify that the dog/puppy does not have any health problems.

A full refund of your purchase price will be made if there are any health issues, which warrant returning the dog/puppy. It must be returned within 24 hours of the examination.  There are no refunds after these dates.

Taylor-Made Chihuahuas is confident that the dog/puppy has been delivered to you free from health problems. From the point the dog/puppy is delivered to me, I am fiduciary responsible as the animals caregiver. The language above describes how I may insure that I have been given a healthy pet.

I am responsible for the cost of the examination. Should a defect be found, I will return the dog/puppy immediately. I know its life depends on that. I will not authorize any treatment or medication, if required Taylor-Made Chihuahuas will take the dog/puppy to their veterinarian. Taylor-Made Chihuahuas will not be responsible for any charges that I may authorize.

I understand that for my refund, in addition to returning the dog/puppy, I am required to have my examining veterinarian document the health defect on clinic letterhead, mailed directly from the clinic to Taylor-Made Chihuahuas.

I understand that injury or fatality caused by abuse or accident, such as dropping, hypoglycemia, starvation, while in my care, voids the possibility of a refund. The veterinarian of Taylor-Made Chihuahuas will have final judgment in this regard.

I further understand that landlord disapproval, allergies, behavior issues, floppy ears, physical characteristics are not valid reasons for a return.

I am aware that all dogs and puppies need vaccinations to protect their health. I will follow my veterinarians advice, via regular checkups, for future disease prevention, such as vaccinations, rabies, spaying and/or neutering,  and preventive dentistry.

If, within one year of the date of this contract, my veterinarian discovers and documents a "quality of life" genetic defect, such as seizures or lameness, Taylor-Made Chihuahuas agrees to replace my puppy with a similar sex and coat from the next available litter.  Veterinarian charges that I have authorized will remain my responsibility.

 I will not give up my Chihuahua to a dog pound or shelter, rather I will make every attempt to find new parents for my Chihuahua. Taylor-Made Chihuahuas may be able to assist in this process, contact us first.  Taylor-Made Chihuahuas will always provide a home for an unwanted Chihuahua.
